These concerns are real, and we do not dismiss them. But the framing is wrong. SLP for MS is health maintenance, not surrender. Early intervention preserves function. Diet modification prevents pneumonia. Communication strategies preserve relationships. The patients who accept SLP early do better long-term than those who wait until a crisis.

Dysphagia: The Most Clinically Important SLP Indication in MS

Up to 40 percent of advanced MS patients develop swallowing difficulty (dysphagia). Aspiration pneumonia – when food or liquid enters the airway – is a leading cause of mortality in advanced MS. Silent aspiration (without the protective cough) is particularly dangerous because patients do not notice it happening.

Home-based SLP evaluation catches dysphagia earlier than waiting for complications. Treatment strategies include:

  • Chin tuck and head turn postural strategies
  • Diet texture modification (mechanical soft, pureed)
  • Thickened liquids when indicated
  • Effortful swallow and supraglottic swallow techniques
  • Strengthening exercises for swallowing muscles
  • Education on signs of aspiration and what to do

Medicare covers home SLP at $0 copay for eligible patients. The 2025 Medicare home health SLP per-visit rate is $205.22 nationally, with no patient copay under Original Medicare.
