Our central office sits in West Palm Beach, and our Palm Beach care team is the longest-tenured part of the agency — many of our nurses, therapists, and aides have been with us for years and have cared for the same MS families over multiple 60-day episodes.
Palm Beach County’s MS population is shaped by two demographic realities: a substantial retiree community of MS patients who relocated from northern states, and a diverse working-age population including significant Hispanic and Haitian Creole communities in the western and central county. Our central office is in West Palm Beach and our Palm Beach intake team is the most experienced in the agency. RN initial assessments are typically completed within 24 hours of a complete physician referral anywhere in Palm Beach County.
Hospital Discharge Coordination in Palm Beach County
We work with all major Palm Beach hospital systems for hospital-to-home transitions: Cleveland Clinic Florida (Boca Raton, Indian River, and the Tradition campus serving Palm Beach), JFK Medical Center, Boca Raton Regional Hospital (Baptist Health), Bethesda Hospital East and West (Baptist Health), Wellington Regional Medical Center, Good Samaritan Medical Center, St. Mary’s Medical Center, Jupiter Medical Center, and Palms West Hospital. Palm Beach summer heat is intense and humidity is high — both are major triggers for Uhthoff’s phenomenon in MS patients. This population has specific care needs that generic home health agencies don’t address well. The medication lists are longer (an MS patient on disease-modifying therapy plus treatments for hypertension, diabetes, and osteoarthritis is the norm). The fall risk is higher because MS gait disturbance compounds with age-related balance changes. The decision-making about disease-modifying therapy is more complex because most DMT trials excluded older patients. Our clinical team is built around this complexity.
