Miami-Dade is home to several leading MS care centers, and skilled home health works best when it complements specialty care rather than running in parallel. We coordinate routinely with:

Hospital Discharge Coordination in Miami-Dade

For patients being discharged home from a hospital admission, we coordinate transitions with all major Miami-Dade hospital systems including Jackson Health System, Baptist Health, the University of Miami Hospital, Mount Sinai Medical Center, HCA Florida hospitals, and the VA system. Bedside intake by our team can typically be completed during the discharge process, with skilled care starting at home the same day or the day after discharge.

We coordinate with the MS Focus Foundation’s Cooling Program for free cooling equipment when patients qualify.

Hurricane season requires real preparation for MS patients dependent on power for mobility devices, BiPAP, refrigerated medications, or hospital beds.
